To prepare children with Special Educational Needs and Or Disabilities to have a successful start to school through effective planning and communication between settings and schools. The purpose of this professional development group will be to focus on how to make transitions timely, well planned and person centred through effective communication between settings and schools.

There will be a focus on what has gone well in the past through effective evaluation so that an early and holistic approach to planning can support a well prepared transition for children and families for entry to school. This will support children with SEND to make a successful transition from an early years setting into a school environment.

The target audience will be early years SENCO and leaders and Reception teachers/leaders and SENCOs
